What are your recommendations for a locking pin back? I see a few types out there. One that requires a tool and others that don't.

These are the best, in my experience. Very easy to pull off, yet it's so hard if you don't know why lol. And I've never had a pin fall off. Don't ask me where to find them though lol, I guess Ebay or something. My mother bought them back in 2002 from a seller in Paris. We have a whole bag of them.

I use this kind as well. I don't know if it's operator error or what, but every so often, I do get one that is a HUGE pain to get onto the pin, but that's maybe only 1% of the time.
 
For 3 years I have just forced the pin post through the black rubber of a mickey back and made a new hole just above/below the original post hole. I have never lost a pin. I check them to make sure they are snug all the time, but I have only had one fall out when the lanyard was in my backpack, never wearing it. Special pin backs are just a waste of money to me. And yes, I wear $300+ single pins on lanyards.
